Does TDU access the DVD drive while you're playing the game or is it just at start up?

You should have the DVD in at all times to play the game.

I do know to keep the dvd in during the game. My question is does the game access the dvd while playing or does it just use it at start up?

It uses it to confirm that you're using a legal copy, then at random times, verifies that the disk is still in the drive. That's why you can hear a "seeking" noise every so often in your DVD drive
