Creating a Poll - Internal Server error

Hi.

 

When I try to create a thread with a poll it keeps giving me an internal server error message and when I go back, the thread has already been created... But without the poll... This is the second time this has happened to me, so I am not sure what the problem is.

 

So is there anything I am doing wrong or is it just a site issue? Also, is there a way I go about avoiding this or fixing the problem after it has occurred??
