There is no easy answer. There are a couple tutorials here. I belive there are tutorials on youtube too

the process is exactly the same for 3D models from any source.

 

If the GTA5 mod is locked, there is no way, however. Ask the creator for permission, he might send you the model. Even if the mod is unlocked you should ask.
